- Your First Visit:
Please plan to arrive 15-30 minutes before your scheduled appointment time in
order to complete necessary paperwork. This will allow us to establish your
medical file and enter key information into our computer system before you see
your doctor.
For your convenience, copies of the required forms can be downloaded from
this website and filled out in advance, saving you some time in our office. Be
sure to download all 3 forms:

- What to Bring:
Please bring pertinent medical information (such as records or reports from
referring physicians), your current insurance card(s), a picture ID, and if
you wish to save time, the completed forms already mentioned.
Payment Information:
We accept many types of insurance. (Click here for a
list of Accepted Insurances.)
Co-payments and payment for services not covered by your insurance will be
due at your first visit. For your convenience we accept cash,
check or credit card (MasterCard, Visa, DiscoverCard, and American Express).
We also offer patient financing through PulseCare, a credit payment plan for
health care services. (PulseCare requires a quick credit check.)
If we are contracted with your insurance carrier, we will file an insurance
claim for the insurance portion of your office visit(s), and make the necessary
contractual adjustments. We also will file claims for surgeries, pregnancy and
subsequent delivery care. Remaining balances will be billed to you.
If we are
not contracted with your insurance carrier, we will provide you with appropriate
receipts so that you can seek reimbursement from your carrier directly.

- Is your Primary Care Physician (PCP) a member of ARIA, PIPA or QCN?
- If your Primary Care Physician is a member of a network of physicians such as ARIA (Austin
Regional Independent Association), PIPA (Physicians Independent Practice
Association), or QCN (Quality Care Network)
the physicians of Drs. Des Rosiers & Wernecke Associates may not be
authorized to see you.

- The physicians belonging to these networks must
generally refer to other physicians within the same network. Unfortunately,
many insurance directories do not make this clear. To avoid frustration at the
time of your appointment, please do the following:

- 1. Contact your insurance company for specific plan guidelines regarding
your access to specialists, particularly if your PCP is with one of the above
networks,
- 2. Ask your PCP if he or she belongs to one of the networks listed above.
If it turns out your PCP is a member of one of these networks, and you
have limited access to specialists (i.e., only specialists within that same
network), you will have to select another OB/GYN office or choose to change your
PCP.

- Flu Shot Information
- This flu season we will be providing flu vaccines for current
patients who are at high risk for complications from influenza as
defined by Centers for Disease Control (CDC).
